How do you make rehearsals productive instead of just running the set?

Running the set is useful, but some rehearsals feel like everyone plays the songs, agrees they were mostly fine, and then the same weak spots are still there next week.

What makes a rehearsal actually productive for your band? Do you isolate transitions, record everything, assign homework, focus on arrangements, or keep a list of problem sections?

@Musician Recording the rehearsal changed everything for us. It removed the guesswork and made weak sections obvious.
